Open vs Closed Rhinoplasty
Understanding the two main surgical approaches
There are two primary techniques used in rhinoplasty: open and closed. The most suitable approach depends on the individual’s nasal anatomy, the complexity of the procedure, and the specific goals of surgery. Both techniques can produce excellent results when performed by an experienced surgeon.
Open Rhinoplasty
In open rhinoplasty, a small incision is made across the columella—the strip of skin between the nostrils—allowing the skin to be lifted and the nasal framework fully exposed.
Advantages:
- Provides greater visibility and access to the nasal structures

Considerations:
- The external scar is usually very small and fades well with time

Rhinoplasty

Septorhinoplasty refers to rhinoplasty surgery that also addresses the nasal septum, the midline wall of cartilage and bone that separates the two sides of the nose. When the septum is bent or deviated, it can block airflow and cause breathing difficulties. In such cases, a procedure known as septoplasty is performed as part of the operation to straighten the septum and improve nasal function.
A deviated septum can also cause the external nose to appear crooked or off-centre. In rhinoplasty, it’s important to correct both the internal structure and the external shape to achieve the best outcome, both aesthetically and functionally.
Correcting a deviated septum involves a range of surgical techniques and requires careful planning, precision, and experience. In fact, most rhinoplasty procedures involve the nasal septum to some extent, whether for structural support, alignment, or improving airflow.
Revision Rhinoplasty
A tailored approach to correcting previous nasal surgery
Revision rhinoplasty, also known as secondary or re-do rhinoplasty, is performed to address concerns that were not fully resolved, or have developed, following previous nose surgery. These may include ongoing issues with nasal shape, asymmetry, or breathing difficulties.
Each revision case is unique and often more complex due to factors such as scar tissue, altered anatomy, or weakened nasal structures. In some patients, structural support must be rebuilt using grafts. This may involve using the patient’s own rib cartilage or fascia, or occasionally donor material, to reconstruct and refine the nasal framework for a more stable and balanced result. Mr Ranganathan will discuss whether grafting is necessary in your case during the consultation.
Given the additional challenges of scarred skin and previous surgical changes, revision rhinoplasty requires careful evaluation, meticulous planning, and a high level of surgical expertise.

A deviated septum may be present from birth, develop over time, or result from injury. In some cases, it may also contribute to frequent nosebleeds, sinus infections, or snoring.
The aim of septoplasty is to reposition the septum to the midline, improving airflow and allowing for more comfortable, unobstructed breathing. The procedure is performed inside the nose, so it usually leaves no visible scars.

Nasal Reconstruction
